Features of the GOA central nervous system - severe bone pain and rapidly progressing course, the disease is often accompanied by gynecomastia.
Radiologically, periosteal layers are detected on the diaphyses, more often in the bones of the forearm and lower leg, the lesions are usually symmetrical. In the blood test, there is often an accelerated erythrocyte sedimentation rate (ESR), an increased concentration of the bone fraction of alkaline phosphatase (ALP), while the rheumatoid factor (RF) is usually absent.
Pathogenesis. It is believed that the development of proliferative processes in the distal bones is associated with the migration of megakaryocytes there, which are capable of secreting growth factors (vascular endothelial growth factor (VEGF), platelet-derived growth factor (PDGF), transforming growth factor (TGF-1), prostaglandin E2, bradykinin, which are normally inactivated in the lungs. All of the above substances lead to local activation of endothelial cells, and also increase the activity of fibroblasts and osteoblasts. The ability of the tumor to secrete hormone-like substances (VEGF, etc.) is considered important in the development of secondary GOA.
GOA is associated with the following tumors: pleural mesothelioma - 50-60%, bronchogenic lung cancer - 10-20%, lung metastases from tumors of other locations, bone metastases from kidney tumors, nasopharyngeal carcinoma, rhabdomyosarcoma, lymphogranulomatosis, thymoma; esophageal leiomyoma, osteogenic sarcoma, fibrosarcoma.
GOA almost never develops in small cell lung cancer.
The symptoms of PNS GOA usually disappear with successful treatment of the tumor. Symptomatic and pathogenetic treatment includes the use of bisphosphonates, opiate analgesics, NSAIDs, inhibitors (tumor necrosis factor (TNF) -), gefetinib (an inhibitor of the epidermal growth factor receptor), VEGF inhibitors and local radiation therapy.
Rheumatoid-like syndrome.
An increased detection rate of neoplasms in patients with rheumatoid arthritis (RA) has been reported in several studies. At the same time, the role of basic therapy in RA (cytostatics) in the development of neoplasms that may occur after several years of using these drugs is discussed. However, rapidly progressing arthritis, similar to RA, may be an early manifestation of a latent tumor.
Common to RA and rheumatoid-like PNS is the development of symptoms over a period of several weeks to several months. Such manifestations as swelling of the soft tissues, limited mobility of the affected joints and morning stiffness come to the fore.
